A. Position Location/Pertinent Information:
Location: Human Services, Inc. (Specific location address to be added once property is located and procured within Chester County.
Department: Residential Services
Employment/FLSA Status: Permanent, Exempt
Employees are required to pass FBI/Criminal/Child/Driver license background checks. Employee must maintain a valid drivers’ license and current automobile insurance. Employee
cannot be on the Federal ineligibility list.
B. Duties/Responsibilities:
- Clinical Duties
- Lead engagement with resident prior to discharge including but not limited to initial meetings, program visits, clinical documentation review and completion of intake documentation.
- Complete psychosocial assessment and service plan for each new resident upon admission and annually thereafter when applicable.
- Provide ADL and IDL coaching for Community Living Coaching staff to assist them in developing their own strategies when engaging with residents in these areas.
- Serve as the point person for collateral contacts and service planning inquiries.
- Liaison between the resident and their treatment team and natural supports as well as any medical and/or psychiatric providers.
- Develop individualized discharge plans that includes a sustainable housing action plan with ancillary supports for each resident.
- Collaborate with the Residential Supervisor and Community Living Coaching staff to develop an individualized community integration plan for each resident.
- Training
- Provide initial and on-going training to new and experienced residential staff in all aspects of the treatment goals and objectives, organizational functionality, and other staff job responsibilities. Mandatory trainings include: HIPAA, Fraud/Abuse, Complaint Management, Incident Management, Recovery/Resiliency, Cultural Intelligence and Inclusion, Sexual Harassment Awareness and Infection Control.
