Housing Initiative Program

A. Position Location/ Pertinent Information:


The Housing Initiative Program is a full-time, exempt position that provides case management, rental assistance for the following: families whose children are about to go into the child welfare placement system due to the family’s lack of housing prevents the children from returning to their parent(s), and transition age youth who are at risk of being homeless due to lack of stable housing. Participants of the program are identified by staff at the Chester County department of Children, Youth, and families.


B. Duties/Responsibilities:


  1. Complete intake. Review intake information, housing issues, financial history, employment, education, psychiatric, substance abuse and medical histories, psychosocial history, and current psychosocial strengths and problem areas.
    1. Develop and periodically review individual service plans for each client on the case load.
    2. Determine need for additional records and information and initiate processes to obtain such records and information
  2. Facilitate individual service plan:
    1. address client needs as determined by service plan
    2. function as advocate of client to access appropriate services
    3. provide initiative in networking and communicating with various service providers, caregivers, landlords involved with the identified client.
    4. provide outreach services to client and involved others that may include homes and community visits, and appropriate settings in which to meet with client and involved others
  3. Document and Supervise Housing Initiative Services:
    1. provide and maintain appropriate documentation of all services rendered in the Housing Initiative Program
    2. complete initial assessment within 30 days of intake and forward to CYF case worker
    3. complete monthly reports on all open cases and forward to Cindy Scott
    4. review client status toward self-sufficiency/compliance with contract
  4. Budgeting/Payment Requests
    1. collect household bills and income verification in order to complete initial budget
    2. when applicable, complete graduated rental payment worksheet
    3. complete and process monthly payment requests by the 25th of each month for ongoing cases; as needed for all others
  5. other duties as assigned


  6. This position is supervised individually or as a group. Work assignments are given by the director of residential and support Services and by the supervisor verbally or in writing. Work performance is evaluated after the first 6 months of employment and annually by the supervisor. Supervision is also provided during office hours on an as needed basis. the Housing Initiative staff will have access to phone supervision during off hours.
