Forensic Outpatient Coordinator

All employees will adhere to Recovery Principles as employees of Human Services, Inc. (See attached document). Staff will participate in annual Recovery training and will utilize Recovery tools in the provision of services such as Personal Medicine, Power Statements, Common Ground , community integration and natural supports.

B. Duties and Responsibilities:

  1. Provide individual, group, and family psychotherapy as needed.
    Standards: Schedule 80% of time. Maintain 70% Direct Service Rate.
  2. Maintain and complete all records, progress notes, treatment plans, progress reviews, insurance authorization forms, and change of status forms in a timely manner to meet applicable State, Federal and insurance regulations. Standard=100%
  3. Provide case management to all clients on caseload as needed.
  4. Provide therapy services during evening hours a minimum of two days per week.
  5. Complete all required in-service training yearly: HIPAA, Fraud, Waste & Abuse, Cultural Diversity, Recovery Model, Complaint & Appeal Process, Incident Management, Sexual Harassment and Infection Control.
  6. Participate in outpatient staff meetings.
  7. Attend continuing education, and participate in evidence based, best practice initiatives as required.
  8. Act as backup for Outpatient staff as needed.
  9. Provide DBT and/or Forensic Outpatient Services as needed.
  10. Provide counseling at satellite offices as needed.
  11. Perform other job-related duties as required.

C. Forensic Job Duties

  1. Complete ASO reports and submit to Chester County Probation and the ASO Board by the 5th day of each month. If the 5th day of each month falls on a Saturday or Sunday, the reports are due by the close of business on the Friday before the 5th day of the month.
  2. Complete SVP report and submit to Chester County Probation, President and CEO and, when applicable, the Clinical Coordinator and/or Clinical Program Manager.
  3. Assure reminder letters are sent for polygraphs and management of payments at the time of polygraph test. Copy of letter should be sent to Clinical Coordinator.
  4. Participate in the ASO meeting on the 3rd Tuesday of every month.
  5. Participate in the weekly Forensics meeting.
  6. Complete behavior alerts for forensic clients and send to Chester County Probation as needed.
  7. Review, schedule and assess intakes for Forensic program.
  8. Other duties as assigned by the Clinical Program Manager and/or President and CEO.
